Volunteer Application

Volunteer Application Information:

We are extremely pleased that you are interested in volunteering to assist our Sylvan Union School District students and staff! Your time, energy, and commitment is greatly appreciated!

In order to volunteer in the Sylvan Union School District, you must complete a Volunteer Application packet. Here are the items that you will need in order to become a volunteer in the district:

  • Level 1: Permission to be on campus for activities including but not limited to observations, meetings, school events, and assemblies. You MUST check in with your valid ID.
  • Level 2: Permission to be on campus and have direct contact working with students either ongoing/weekly in small group or classroom setting. You MUST check in with a valid ID and completed the volunteer application process. (ID, TB Clearance, Site Training, Cleared by Admin)
  • Level 3: Permission to be on campus and have direct contact working with students either ongoing/weekly in small group or classroom setting, or chaperoning an approved field trip. You MUST check in with a valid ID and completed the volunteer application process. (ID, TB Clearance, Site Training, Cleared by Admin, Live Scan Fingerprints by DO, and Mandated Reporter Training)

Volunteer Training Dates:

  • The training dates are temporarily closed for the 2019-2020 school year. Please look for new training dates in August 2020.

Please return the completed application to the school site in which you would like to volunteer.

You will then need to bring a copy of the completed volunteer application to the Sylvan Union School District Office (605 Sylvan Avenue) to receive a Live Scan request form (at no cost to the volunteer).
